2012 Brochure & Recommendation Form

OPHB 2012 will take place from October 31st to November 3rd, 2012.
To be eligible for the Ontario Provincial Honour Band 2012, the student must be enrolled in a Secondary School for the year 2012-2013.
Please include audition recordings with this recommendation form. Instructions for student auditions are available on the Audition Requirements page.
The appropriate participation ($250 or $275, + a separate $10 non-refundable application fee) should be submitted with the application package. Make cheques payable to Ontario Band Association. Unsuccessful applicants' cheques will be returned (minus application fee).
There may be a limited number of entrance scholarships available to help participants offset travel expenses. Be sure to indicate the applicant's interest in a scholarship at the bottom of the form.
